      *  Physical - must have a current physical on file and needs to

                       include the KHSAA High School consent form

​

      *  Emergency Form - important information for the HHS medical

                       staff & coaches

​

      *  Release of Health Information - this form is required

                      by St. Elizabeth Healthcare and must be turned in for

                      for your child to recieve treatment from the the St.

                      Elizabeth athletic trainer on staff. A new form is  

                      needed every school year
